Frequently asked questions about appointments.

 

May I schedule my own appointment or do I have to be referred to your group?
You may schedule your own appointment unless your insurance requires you to get a referral from your primary care physician before you see a specialist.

How do I know if I need a primary care referral?
You may view your insurance card or packet to be sure you are familiar with the re¬quirements of your particular plan. You may also call the phone # listed on your card and speak to a customer service representative.

What should I bring to my appointment?
Any previous films, reports, test results and records pertaining to this appointment. Insurance cards to copy and co-payment as required by your insurance plan. Comfortable clothing to allow for proper examination. Gowns or shorts will of course be provided if necessary. List of medications/dosages, allergies, dates of previous surgeries to help you in completing your personal medical history form which you may also download, complete at home and bring with you.

What is your cancellation Policy?
As a courtesy to other patients and the Physicians, we kindly ask that appointments be cancelled a least 24 hours in advance.

What is a first report of injury?
Workers' Compensation Commission Form WCC-15 (for non-State employees) or State of Connecticut Form PER WC-207 (for State employees) required to be filed by an employer in cases of an employee's occupational injury or disease that results in incapacity from work of one day or more.
